As this programme will be running during the time that the school is closed, weekly payment in full is required prior to your child attending the programme. Payment for the holiday programme is required 2 weeks prior to the programme for staffing purposes. Payment is via our Internet Banking or Cash/Eftpos during term time.
Non-payment will nullify your child's enrolment in the remainder of the Holiday Programme.
Mayfair OP Holiday programme is OSCAR approved. Subsidy Application forms are available from the School Office. If you require a WINZ subsidy to support attendance, please discuss this with the Supervisors.

SETTLEMENT OF ACCOUNTS:
All payments made will be checked weekly. The service relies on the funding generated and any missed payments may result in loss of services.
DEFAULT & CONSEQUENCES OF DEFAULT
BEHAVIOUR
Children will be expected to have the same behaviour standards as they do during school hours. The supervisor will be responsible for the children's behaviour and so children will be expected to respond to requests made by the supervisor. Failure to do so could well mean the child/ren will not be able to attend the Mayfair OP.EQUIPMENT
There will be a range of activities for the children to take part in. Equipment is to be cared for and retained at school. Children will be expected to assist with tidying up at the end of each day.
ATTENDANCE
Supervisors will assume children are attending on each day they are enrolled for. If this is not the case, please advise them on the day before the programme starts. The supervisor will keep a register of attendance including the hours the child attends each day, and follow up with caregivers when children do not arrive. Alternatively, a phone call to the supervisors if you require last-minute care, will also be acceptable.
POLICIES
It is a service provided as an extension to our normal school day. In light of this, the policies that the BOT has in place for the running of the school are the same policies that apply to Mayfair OP.
DROPPING OFF AND PICKING UP CHILDREN
Parents/Caregivers are asked to sign their child out after each session. Parents/Caregivers will need to use the Aimy Plus application on ur Mayfair OP device to sign in/out any children as this will avoid any issues of children not being accounted for and ensure the safe, secure environment for the children is maintained.
PARENT CODE OF CONDUCT
To ensure a safe, respectful, and supportive environment for all children, staff, and families, all parents and caregivers attending or interacting with the Mayfair OP programme are expected to follow this Code of Conduct:
Respect for Staff and Children
Parents/caregivers must speak respectfully to all staff and children. Verbal abuse, rudeness, raised voices, swearing, threats, or any aggressive or inappropriate behaviour will not be tolerated. Any such behaviour may result in your child being withdrawn from the programme.
Timely Collection of Children
Children must be collected by the advertised finishing time. Being consistently late places a burden on staff and disrupts the programme. Please ensure you are on time.
Late Pick-up Fees
Late pick-ups will incur a fee of $10 for every 10 minutes (or part thereof) past the programme’s finish time. This charge is the responsibility of the parent/caregiver and must be paid before your child can attend another session. Ongoing late collections may result in a review of your child’s enrolment in the programme.
Authorised Collection Only
Children will only be released to authorised adults listed on the enrolment form. If someone else is collecting your child, you must notify the programme supervisor in advance.
Supporting Programme Rules
Parents/caregivers are expected to support the programme’s expectations, staff, and procedures. If you have concerns or feedback, please approach staff respectfully and follow the programme’s complaints process if required.
Non-compliance with this Code of Conduct may lead to verbal or written warnings, a meeting with management, or removal from the programme depending on the severity of the behaviour.
PRIVACY ACT 2020
The information that you have supplied is necessary for the safe and effective operation of the Mayfair OP. All personal information requested will be destroyed at the completion of your child's time in the programme. You are welcome to review information pertaining to your child's enrollment at any time.
By signing below, you authorise Mayfair OP to:
collect, retain and use any information about you for the purpose of assessing your creditworthiness or marketing products and services to you: and
disclose information about you, whether collected by Mayfair OP from you directly or obtained by Mayfair OP from any other source to any other credit provider or any credit reporting agency for the purposes of providing or obtaining a credit reference, debt collection or notifying a default by you.
Where you are an individual the authorities under this clause are authorities or consents for the purposes of the Privacy Act 2020. You shall have the right to request from Mayfair OP a copy of the information about you retained by Mayfair OP and the right to request Mayfair OP to correct any incorrect information about you held by Mayfair OP. It is the caregiver's responsibility to inform Mayfair OP of any parental/custody orders, which may affect the children during their time at the Mayfair OP.
Terms of Trade
By signing this form you have indicated that you have received, read and agree with the above and will abide by the conditions as outlined.
I agree to pay the fees as stated, have read the Complaints procedure, and agree to the important information as stated on this enrolment Form.
I give permission for my child/ren to attend any off-site activities arranged and supervised by the Mayfair OP supervisor and staff. I also give permission that in the event of a medical mishap occurring, medical assistance will be given as appropriate and I will be informed as soon as possible. I understand it is my responsibility to keep the Supervisors informed with up-to-date medical details.
